Clinical Realities and Considerations for Aesthetic Zone Implant Therapy

In the partially edentulous patient, achieving functionally acceptable treatment outcomes is often realized.  However, when tooth replacement is required in the aesthetic zone, the additional challenge of meeting both patient and clinician aesthetic expectations may prove to be an exponential one.  The purpose of this presentation is to discuss the literature and clinical realities of dental implant therapy and complications and how we can apply what we learn toward improved aesthetic zone treatment planning and clinical procedures. 

Upon completion of this presentation, participants should be able to:
1.Understand common complications and pitfalls, including abutment selection and cementation challenges.
2.Have an improved understanding of dental implant treatment planning in the aesthetic zone.
3.Discuss provisionalization and immediate provisionalization alternatives.
4.Discuss current workflow utilizing guided surgery to help achieve improved outcomes.
Dr. Leopardi received his undergraduate dental degree from the University of Adelaide, South Australia, in 1984.  He enjoyed private practice as a general dentist in Adelaide, Australia, for several years prior to receiving his specialist training and masters degree in combined fixed, removable and implant Prosthodontics, at the University of North Carolina, at Chapel Hill, in 1993.

At Chapel Hill, he also taught Fixed Prosthodontics as a Clinical Assistant Professor.  Upon return to Australia, he began a successful Prosthodontic private practice in the Gold Coast region of Queensland, Australia.

In 1996, he joined the Prosthodontic faculty at the University of Detroit Mercy (UDM), Michigan, where he also obtained his American DDS degree.  Teaching responsibilities at UDM included Clinical Supervisor and:

  • Co-Director of the Fixed Prosthodontic Program,
  • Director of the Complete Dentures Program,
  • Implant Residency program supervisor.

In 1999, after 15 years of combined academic and dental practice, Dr. Leopardi resumed private practice limited to implant, fixed and removable Prosthodontics in Denver, Colorado. Today he lectures nationally on subjects involving fixed, removable and implant dentistry, and is involved in clinical research.  He is also the founder and President of the Denver Implant Study Club and The Knowledge Factory.

Surgically Facilitated Orthodontic Therapy (SFOT). An interdisciplinary approach

Patients that want more ideal facial esthetics and function, may choose to have the interdisciplinary team utilize SFOT to alter the dentoalveolar bone and optimally position the roots of the teeth for the patients' face. Space may be appropriated to enable the restorative dentist to create natural tooth dimensions and morphology in the worn, eroded or otherwise altered dentition. Surgical, orthodontic and restorative perspectives will be presented to challenge the team to move beyond traditional interdisciplinary therapies into a new frontier for enhanced outcomes.

Educational Objectives:

  • To appreciate dentoalveolar bone phenotypes and to limit associated risks using SFOT
  • To review diagnostic findings and imaging modalities critical to treatment planning of the interdisciplinary patient
  • To present the biology, risks/benefits and execution of SFOT in advanced interdisciplinary cases from a surgical, orthodontic, and restorative perspective
  • To discuss SFOT to optimize the position of teeth for facial esthetics and function in the skeletally mature patient with "grey zone" malocclusions 
Dr. Mandelaris attended the University of Michigan from undergraduate through dental school. In 1999, he completed a three-year post-graduate residency program in Periodontology at the University of Louisville, School of Dentistry, where he also obtained a Master of Science (M.S.) degree in Oral Biology.  Dr. Mandelaris is a Diplomate of the American Board of Periodontology and has served as an examiner for Part II (oral examination) of the American Board of Periodontology certification process.
He has served as an Assistant Clinical Professor in the Department of Oral and Maxillofacial Surgery at Louisiana State University, School of Dentistry (New --Orleans, LA) and, currently, is an Adjunct Clinical Assistant Professor in the Department of Graduate Periodontics at the University of Illinois, College of Dentistry (Chicago, IL). He is also a Fellow in both the American and International College of Dentists.
Dr. Mandelaris serves as an ad-hoc reviewer for the Journal of Periodontology, the International Journal of Periodontics and Restorative Dentistry, and the International Journal of Oral and Maxillofacial Implants. He has published in several peer-reviewed journals such as the International Journal of Periodontics and Restorative Dentistry, Compendium for Continuing Education in Dentistry, Practical Procedures and Aesthetic Dentistry, the Journal of Periodontology, Clinical Advances in Periodontics, and The Journal of Oral and Maxillofacial Surgery. He has also contributed several chapters in five different textbooks used worldwide on subjects related to computer guided implantology, CT diagnostics and, most recently, Surgically Facilitated Orthodontic Therapy (a book he edited and which he authored the chapter on surgical therapy). He is a Past President of the Illinois Society of Periodontists and has served on several committees for the American Academy of Periodontology. In 2014, he was one of 52 periodontists worldwide selected to participate in the American Academy of Periodontology’s commissioned Periodontal Regenerative Medicine World Workshop on Emerging Regenerative Approaches for Periodontal Reconstruction.

A nationally recognized expert, he was appointed by the American Academy of Periodontology in 2015 to co-chair the best-evidence consensus workshop on the use of CBCT imaging in Periodontics as well as co-author the academy guidelines.  He holds memberships in many professional organizations, most recently including the American Academy of Restorative Dentistry and American Society of Bone and Mineral Research. Dr. Mandelaris is in private practice at Periodontal Medicine & Surgical Specialists, LTD in Park Ridge and Oakbrook Terrace, Illinois.
