Chief Complaint:  “I need a crown placed on my implant and I don’t like my smile.”
History:  Twenty-three year old male presented as a referral from his periodontist with implant #7 already in place.  History of trauma to #7, 8 and 9. 
Clinical Exam:  Integrated implant #7 with acceptable soft tissue volume.  Endodontically treated #8 with composite repair.  MI fracture #9. 
Diagnosis:  Unrestored implant #7.  Compromised and esthetically displeasing #8.  Class IV fracture #9. Developing papilla deficiency between centrals and improper gingival architecture around implant.  Otherwise, acceptable health and function.

Case Write-up:

Treatment:  Completed diagnostic wax up, following diagnostic photography, casts and patient feedback.  Impacted changes in tooth contour, length and gingival management.     
Final Restorations:  #7 - Custom Zirconia Abutment.  E-Max all-ceramic crowns #7 and 8.  MIFL bonded composite #9.
    High level of artistic detail was paid to fabricating an esthetic presentation which was both pleasing and naturally appropriate.  Anatomical contour and overall tooth arrangement, staining, value, translucence, gingival form and architecture were specifically designed to blend with the patient’s natural dentition.  Demonstrates the result which can be achieved when clinician and technician work closely together for functional and esthetic success.
