Chief Complaint: ìI am concerned I am going to lose my upper teeth√¢ÀÜ≈°√É‚Ä∞√¢À܂Ć√ɬ≠√¢ÀÜ≈°√É≈ì√¢‚Ǩ≈°√É‚Äû√ɬ¥√¢ÀÜ≈°√É‚Ä∞√¢‚Ǩ≈°√É‚Äû√ɬ∂√¢ÀÜ≈°√ɂİ√Ǭ¨√É‚Äâ€ÂÂ
History: Fifty-two year old patient presents with failing fixed partial denture #3-14, as a referral from her oral surgeon. Existing restoration is approximately 27 years old.
Clinical Exam: Failing abutment teeth due to recurrent decay, endodontic failure and periodontal bone loss.
Diagnosis: Esthetically displeasing maxillary fixed partial denture. Maxillary abutment teeth with poor or hopeless prognosis. Hard and soft tissue loss.
Treatment: Extraction of all maxillary natural teeth. Placement of seven Nobel BioCare Replace Select dental implants. Maxillary interim complete denture, as provisional.
Final Restoration: Titanium custom abutments #2, 4, 5, 7, 10, 12, and 15. Metalceramic fixed partial denture #2-15 with pink porcelain gingival tissues. Jensen Foundation Alloy. Creation Porcelain. Passive fit on all implants was achieved by using galvanoformed gold copings for each abutment. Final pick up of copings within the restoration framework was completed following all porcelain application. Screw retention using horizontal set screws on #5 and #12 allowed predictable retrievability and weak cementation on other custom abutments.
Final prosthesis accomplished goal of providing a fixed, durable, functional, hygienic and comfortable restoration. High level of artistic detail was paid to fabricating an esthetic presentation which was both pleasing and naturally appropriate. Anatomical contour and overall tooth arrangement, staining, wear, value, translucence, gingival form, color and architecture were specifically designed and age appropriate. Demonstrates the result which can be achieved when clinician and technician work closely together for functional and esthetic success, with technician having the opportunity to directly evaluate and interact with patient.
